Requirements

  • Recommendation Forms A & B (issued by the Guidance and Testing Center after entrance exam)

  • Photocopy of Passport (if non-Filipino)

Entrance Exam Fee - Php 150.00
Minimum Downpayment - Php 5000.00

    *If you were able to secure a slot with Php 2000.00  reservation fee, you just need to add Php 3000.00 to complete the minimum downpayment 
    *Balance to be divided in four (8) equal monthly installment.

SENIOR HIGH SCHOOL ADMISSION REQUIREMENTS AND PROCEDURES

Requirements

RESERVATION PHASE 
You may reserve a slot for LPU Senior High by submitting the following requirements and paying a non-refundable reservation fee worth Php 2000.
  • Entrance Exam Result
  • NCAE Result
  • 4 pcs. Identical 2x2 ID Photos White Background with nametag
  • Photocopy of PSA Authenticated Birth Certificate
ENROLLMENT PROPER
Reserving a slot is not equivalent to enrollment. Students and parents are requested to come back to complete enrollment starting April 2017 with these requirements:
  • Admission Stub with Receipt
  • Original Report Card
  • Form 137
  • Original Certificate of Completion
  • Recommendation Forms A & B (released by LPU Guidance and Testing Center during admission exam)
Entrance Exam Fee - Php 150.00

PROCEDURES
  1. Fill out the Application Form.
  2. Proceed to the Cashier for Entrance exam payment.
  3. Present the Official Receipt and Application form to the Guidance Counselor/Staff on the day of the examination.
  4. Reservation
  5. Proceed at the High School Department to Register.
  6. Pay the corresponding Reservation fee at the Cashier (Php2000.00) (deductible from the total fees).
  7. Return at the High School Department to finalize your Reservation.
  8. OFFICIAL SHS ENROLLMENT
  9. Submit the following to the Admissions Office:
  • Entrance Exam Result (Original)
  • National Career Assessment Examination (NCAE) Result (Original copy)
  • Four (4) pieces identical 2×2 ID picture (recent) with white background and nameplate
  • Photocopy of Applicant’s PSA birth certificate (authenticated)
– If applicant has dual citizenship, submit a photocopy of Philippine passport or Certificate of Recognition as a Filipino Citizen
– If applicant is not a Filipino citizen, submit a photocopy of Alien Certificate of Registration (ACR) and a Special Study Permit issued by the Bureau of Immigration
  • Reservation Receipt (if available)
  • Original copy of Report Card from High School (Form 138)
  • Original Form 137-A / Student Permanent Record (if available already)
  • Original Certificate of Completion (Grade 10)
  • Recommendation Forms A & B
Proceed to the Principal’s office for the interview. (Both parent and applicant will be interviewed).
  1. Proceed to the Cashier for the Tuition down payment. Return to the High School Department for the releasing of the Official Registration form and Enrollment Kit.
  2. Proceed to the Office of Student Affairs and Services for I.D. processing.
  3. Proceed to Bookstore for the measurement and ordering of uniform.
  4. Please take note of parent and student orientation schedules, claiming of books, etc.
  5. Proceed to the Principal’s office for the interview. (Both parent and applicant will be interviewed).
  6. Proceed to the Cashier for the Tuition down payment.
  7. Return to the High School Department for the releasing of the Official Registration form and Enrollment Kit.
  8. Proceed to the Office of Student Affairs and Services for I.D. processing.
