Senior High School Applicants

GENERAL REQUIREMENTS FOR APPLICATION
  1. Photocopy of the latest report card (F-138) with the Learner Reference Number (LRN) indicated
  2. Photocopy of PSA/NSO birth certificate
  3. Two (2) copies of 1x1 ID picture
  4. Acknowledgment Slip of the Bulletin of Information for Admission (may be secured from the Registrar's Office upon submission of the first 3 requirements)
  5. Grade requirements: at least 80% in English, Filipino, Science, Math and A. Panlipunan in all quarters and conduct grade of 85% in all quarters
APPLICATION PROCEDURE
  • Submit the complete requirements to the Registrar's Office for evaluation. Please take note that incomplete requirements will not be processed.
  • Accomplish the application form.
  • Pay the entrance examination fee at the Business Office
  • Present the receipt of payment at the Registrar's Office to secure the exam permit and entrance exam schedule.
