- Application Form
- Correctly and completely accomplished Application Form
- Photocopy of Birth Certificate issued by the NSO (bring original copy)
- Photocopy of Form 138 duly authenticated by the School Principal (bring original copy)
- Self-addressed and stamped envelope for the mailing of the examination result
- Two (2) pcs. identical 2x2 recent pictures, colored with red background & name tag
- Fees/Charges: PHP 180.00
Additional Requirements
Fresh High School Graduates/Stopped After High School
- Photocopy of Form 138 (bring original copy)
Transferees
- Official Transcript of Records/Official Certification of Grades from the Registrar (bring original copy)
- Endorsement Letter from the College, signed by the Dean
Returnees/Shifters
- Appraisal Sheet/Report of Rating/ Official Certification of Grades from the Registrar
PEPT/ALS – A&E Eligibles
- Proof of Eligibility for College Admission
Procedures
- Get Application Form and List of Requirements at the Testing Services
- Fill up Application Form
- Proceed to LS Guidance Office for initial assessment of documents
- Proceed to Testing Services for final processing of documents
- Pay the prescribed testing fee at the Cashier’s Office
- Submit Official Receipt at the Testing Services for the preparation of test permit
- Receive test permit
- Take the test on the scheduled date
- Release of test result on date of scheduled
- Secure Application Form at the Testing Services
- Fill up Application Form
- Submit Application Form and Requirements at the Testing Services for evaluation
- Undertake interview (Transferee, Shifter, Returnee, Stopped After High School)
- Pay the prescribed testing fee at the Cashier’s Office
- Submit Official Receipt at the Testing Services for the preparation of test permit
- Receive test permit
- Take the UNP-CAT on the scheduled date
- Wait for the release of test result
Batch Processing (For the School Representative)
- Organize the students from his/her school who are planning to apply to UNP
- Explain to the students how to fill-up the Application Form and what requirements to prepare
- Collect the Application Forms and Requirements of the students. Affix his/her signature at the end of the Application Form after checking its completeness and correctness
- Arrange all the Application Forms in alphabetical order
- Make an alphabetical listing (Master List) of the applicants. The said list should indicate the individual as well as the total amount to be paid by the group as a whole
- Ensure that both he/she and the applicant counter-sign opposite the applicant’s name whenever a payment is made
- Ensure that the School Principal/ Head signs in the Master List
- Submit Application Forms, Master List, and Requirements at the Testing Services for evaluation
- Pay the prescribed testing fee at the Cashier’s Office
- Submit Official Receipt at the Testing Services for the preparation of test permits
- Receive and ascertain completeness of test permits
- Advise students to take the test on scheduled date
- Inform students to wait for release of test results
