Admission Requirements and Procedures - Graduate School

Requirements

Procedures
  1.  Secure Admission Requirements from the Graduate Programs Office.
  2.  Go to the Guidance and Evaluation Center for schedule of Entrance Examination.
  3.  Upon completion of requirements, the student will be informed for an interview with the Dean.
  4.  Upon admission, the student will be required to fill-up the ID issuance form.
  5. After issuance of ID no., the student can proceed with the enrolment.

Enrollment
  1. Academic Evaluation/Electronic Advising by the Dean.
  2. Proceed to the  Business Office for payment.
  3. After payment, proceed to the Graduate Programs Office for Reservation, Enrolment Printout  and Validation of Enrolment Form.
  4. ID Issuance/Renewal
  • For continuing students, stickers will be provided during the first 2 weeks of classes
  • For new students, please proceed to Instructional Media Center for ID capture.

  • Accomplished Recommendation Form B.
  • Junior High School Report Card / Form 138 (Original) with LRN number

Note: Average for STEM Strand in (English, Science, and Mathematics) is 80.
 
  • National Career Assessment Examination (NCAE) Result
  • NSO Birth Certificate
  • ESC Certification from school principal (for private ESC recipients only)
  • QVR Certificate (for private school qualified recipients only)

Procedures
  1. Present SHS Admission Card to the Satellite Cashier and pay the remaining upon enrolment fee (4,000).
  2. Proceed to ASAO for submission of requirements
  3. Submit the said requirements.
  4. Printing of Enrolment Form
  5. Signing of Enrolment Form
  6. Submission of Enrolment Form for validation (Student’s copy will be provided

Requirements

Eligibility for Application

Freshmen


An applicant must be any one of the following:
  1. A Grade 12 student expected to graduate at the end of Academic Year 2018-2019;
  2. A Grade 12 student who graduated from a school based abroad prior to AY 2018-2019 and who has not taken any college course;
  3. A 4th Year High School student from a school in the Philippines who graduated prior to AY 2018-2019 and who has not taken any college course;
  4. Passer of the Philippine Educational Placement Test (PEPT), Alternative Learning System (ALS) or Non-Formal Education Accreditation and Equivalency (NFEA&E) Program following DepEd regulations and therefore certified eligible for admission to college/tertiary level;

Returnee

If you have stopped and have not previously enrolled in any university or college upon returning to La Salle Bacolod, please follow the steps below; otherwise, please see the Transferee section:
  1. Secure clearance form at ITC and have it signed by respective offices. Student Accounts issues the returnee clearance slip upon completion of clearance form.
  2. Present Returnee Clearance Slip at ASAO to claim your admission card.
  3. Activate your USLS account at the Registrar’s Office Window 6.
You can now proceed to Enrolment.


Cross-Enrolllee
  1. Check the availability of subject/s with the College you wish to enroll in.
  2. Pay P 200.00 as admission fee at the Cashier’s Office
  3. Submit the following admission requirements
    • Certificate or Permission to Cross Enroll from your current school with the Registrar’s signature and school seal (1 original and 2 photocopies)
    • Electronic Application Form 
    • Undertaking Form (please attach latest 1x1 ID photo with white background)
    • Medical Certificate from USLS Clinic (please prepare 2x2 ID photo with white background)
       
  4. Interview with the Admitting Officer

Pre-Admission Requirements

Below are the requirements that should be submitted through personal visit at Admissions and Scholarships Administrative Office (ASAO) or through email at asao@usls.edu.ph for evaluation purposes:
  1. Letter of Intent
  2. Original High School Report Card and Diploma (for freshman applicants)
  3. Original Transcript of Records with Course Descriptions of subjects (for transferee)
  4. Foreign Student Personal Information Sheet

Applicants who qualify will be advised to follow the admission procedures.


Admission Procedures

STEP 1: ENTRANCE TEST. Foreigner’s test schedule is available at Guidance and Evaluation Center (GEC), ground floor at Benilde Building, beside Nursing Cafeteria.
The following requirements are to bring:
  • 2 pcs 2x2 photo with white background
  • P 750.00 - testing fee

STEP 2: ADMISSION. Two (2) sets of requirement shall be submitted to ASAO for the following:
For Admission
For Issuance of Admission Clearance:
  • Original passport
  • photocopy of bio-page
  • photocopy of latest arrival
  • photocopy of authorized stay

All non-English documents must have an English translation and should be authenticated by the Philippine Foreign Service Post in their country of origin. Non-English speaking applicants must take an English proficiency test and result be submitted at ASAO.

The University of St. La Salle (USLS) is a private Catholic Institution founded by three American brothers namely Br. Dennis Ruland FSC, Br. Hugh Wester FSC, and Br. V. Felix Masson FSC in 1952, and it was initially named as the La Salle College-Bacolod. The college was elevated to its university status in 1988, opening its Colleges of Engineering, Education, Nursing, Law, as well as its Graduate School.

The Colleges of Arts and Sciences, Business, and Accountancy and Education obtained Level III accreditation from the Philippine Accrediting Association of Schools, Colleges and Universities (PAASCU). USLS also has an Integrated School that caters students from Kindergarten to Grade 10. It also has a Senior High School department that offers Academic with ABM, STEM, HUMSS and GAS strand; Sports, Arts and Design, and Technical-Vocational Tracks.
